I can't tell what's causing this. It doesn't happen every time, but about half the time.
There are two ways this issue happens, after I click the rating for the card:
The card stays in place, with the answer visible, but with the Return. button in the upper left. If I click Return. the next card is shown.
The card is prompted again for review, but if you look in the references on the right, the schedule has been actually updated. You can skip the card, but this makes the card count go down a second time.
