"error during init" for a single module easy to miss, and CI still passes

See screenshot. If one module fails for some reason, it would be easy to miss this, especially if other modules have plans in the same comment.

It would be great if it could have some big obvious warning at the top of the comment like "⚠️ warning: some modules failed to init, see below for details".

The ability to customise the comment template could also work here, but I think this would also be sensible to include in default template.

Alternatively if the job failed and went red this would also draw attention to achieve something similar.
Otherwise we are likely to miss these kind of errors.
