What are the attributes required to identify a person ?

For planning and reporting, demographic profile, location etc can be of use.

If any special care is to required, this can be carefully captured without capturing personally identifiable medical conditions or medicine information.

We cannot store items like Aadhar ID but phone number, name etc can be stored.
